Sustaining dementia friendly communities In 2015, we were commissioned by HealthWatch to suggest how dementia-friendly communities could be sustained. We recommended that GPs need to refer dementia patients to local community support groups; health and social care commissioners should recognise that community support can help people with dementia live well at home; and formal referral systems are needed to connect up GPs and community support groups with the professional dementia services provided by the County Council and NHS.
Clinical Commissioning Group: We are members of the County Council’s Adult Social Care Work Force strategy group. And we are members of a group of charities invited by the Clinical Commissioning Group to work up new ideas to strengthen community support for elderly and vulnerable people.
Village Companies: The Village Company is a locally owned social enterprise (serving clusters of villages) that will deliver domiciliary care (and other core services) using a mainly local workforce. We are working with 2 other Rural Community Councils to trial this new approach in a small number of locations in Oxfordshire, Gloucestershire and Nottinghamshire.
